Please note, you are not going to be able to schedule OR CONFIRM YOUR appointment until the following intake paperwork is completed:


  1. Informed Consent to Treat and Patient Bill of Rights and Responsibilities
  2. Late Cancellation and NO Show Policy and Credit Card Authorization  
  3. Clinical history form - Biopsychosocial assessment
  4. Insurance Information Form
  5. Client Information Form including Emergency Contact Information Form
  6. Assessment Scales 

After we verified all your information and reviewed your biopsychosocial form, the initial assessment begins.


Prior to your scheduled appointment, the virtual assistant will be sending you Assessment Scales to determine current severity of Symptoms:  

Validated clinical measures provides objective measures of clinical change to assess improvement or continuing decline in functioning which is helpful for your individualized treatment planning. 


AUDIT ALCOHOL USE

PHQ-9 - assessment of underlying major depressive disorder

GAD-7 - assessment of underlying generalized anxiety disorder

ISI - assessment of the current severity of chronic insomnia

ASRS - assessment of any underlying ADHD/focus/concentration issues

MDQ - assessement of underlying mood disorder

PCL-5 - assessment of underlying trauma symptoms


LATE CANCELLATION AND NO SHOW POLICY:


Due to the high demand of Mental Health Services (MHS), we have a strict late cancellation and no show policy so we can offer that reserved appointment time slot to another patient.


In order for Access Psych Meds to continue to provide access to care, patients are expected to cancel and reschedule appointments 48 hours in advance, otherwise your credit card on record will be charged automatically.


Our goal is to provide prompt access to psychiatric services to our clients


⋙ Initial Encounter (60 minutes appointment) please allocate 75 minutes ⋘

  • Late Cancellation and NO Show Policy - $200 will be automatically charged on your credit card on file. 
  • The clinician will wait for 20 minutes and if the patient didn't show up within that 20 min time period this session will be considered a No Show.  

⋙ Established patient follow-up encounters ⋘

  • Late cancellation and NO show - $80 Credit card charge
  • The clinician will wait for 10-15 minutes and if the patient didn't show with this time frame, this session will be considered a NO Show.
