1. Honest and Full Disclosure of Past History
To provide the best possible care, we require a comprehensive assessment that includes a complete health background. Please share accurate information about past medical conditions, hospitalizations, any thoughts of self-harm or suicide attempts, previous medications (including dosages and side effects experienced), substance use history, and other factors relevant to mental health. This thorough evaluation enables us to develop the most appropriate and effective treatment plan for our patients.
When the patient intentionally withheld information, it is the patient’s responsibility to accept the consequences of these decisions if complications occur.
2. Treatment Recommendations
It is the patient’s responsibility to follow up on the provider’s instructions, take medications when prescribed and ask questions concerning his or her own health care that he or she feels is necessary.
Patients are responsible for their actions if they refuse treatment or do not follow the provider’s instructions and recommendations.
3. Inappropriate Behavior / Termination of Session
Access Psych Med providers have the right to terminate the session prematurely if the patient is being inappropriate and abusive. Patients will be charged the normal session even if the session is terminated prematurely due to inappropriate behavior.
4. Service Fee
Additional fees apply for administrative services such as medical accommodation letters, FMLA documentation requests, medical record copies, and similar requests.
