2. Medical Records from Primary Care Physician, if required by the provider 

Patients are kindly advised to complete the intake paperwork, including consent forms and the client portal, within 72 hours, following communication from the assigned virtual assistant.


To ensure the smooth continuation of your reserved appointment schedule, we kindly request your timely response within 24 hours.


We encourage you to reach out to us for any necessary adjustments to your appointment, whether it be rescheduling, cancellation, or termination of services, with at least a 2-day notice. Your cooperation is greatly appreciated.
