- Comprehensive history taking - medical, mental health, social and family history
- Assignment of instrument scales to identify severity of current symptoms which serves as an aid in treatment planning.
- Followed by clinical interview of our experienced mental health provider
- Diagnosis and Evaluation
- Treatment planning
- Goal Setting
- Prescription Medication (if needed)
- Mandatory paperwork intake requirements
- Informed Consent to Treat
- Late Cancellation and No Show Policy
- Credit Card Authorization for Late Cancellation and No Show Policy
- Comprehensive Intake Questionnaire
- Assessment scales to assess current severity of symptoms
- In order to maintain access to care we have a strict last minute cancellation and NO show policy
- 48 hours cancellation and reschedule policy
